Type 2 Diabetes Primary Care and Referral Pathway

This is a Connecting Healthcare in Communities (CHIC) funded project supported by the Southside Partnership Council

This primary care and referral pathway was developed in consultation with the CHIC Diabetes Project Clinical Advisory Group, which comprised GPs, practice nurses, private allied health providers, Credentialled Diabetes Educators (CDEs) from QEII diabetes service and the Queensland Diabetes Centre at the Mater and an endocrinologist. It maps an evidence based pathway for structured, systematic care for T2D that encourages general practice to undertake more responsibility for the initial and ongoing management of adults with T2D. The pathway shows how to build capacity for structured, systematic, T2D care and supports the development of patient self management skills.

Each element in the process is linked to relevant Medicare items, supporting services, and supporting resources including clinical decision tools and standard, quality managed, patient, self management information and education that is consistent across the network. The pathway also links with a Register of local Health Providers , formally aligned with the T2D Project. It is expected that participation in this network will facilitate better communication and linkages between general practice, allied health and specialist services within a local geographic area.
