Chronic Disease Management MBS Items for Care Planning and Specialist Referral
These items make it easier for GPs and PNs to manage the health care of patients with chronic medical conditions, including those patients who need multi-disciplinary care. For patients requiring multi-disciplinary care, GPs can also claim from Medicare for co-ordinating Team Care Planning and review services. Patients on Team Care Arrangements can access a maximum of 5 allied health services per calendar year.
