Max Howard is currently a private business consultant working within both the private and government sectors. Max currently is the chair of the Logan/Beaudesert Health Community council in Metro South, he also holds board memberships within the Office of the Health Practitioner Registration Boards, member of the Specialist Outpatient Advisory Committee and is in his 14th year as a member of the management committee of the Crestmead Community Centre.
Max is a graduate of the Sunshine Coast University finishing with a Masters of Business Administration; earlier university qualifications also include Informatics, welfare economics and Built Engineering.

David has a physiotherapy background, with 15 years experience working in many roles both public and private, in both Queensland and the UK.
David began leading chronic disease teams 3 years ago and is passionate about improving the management of chronic disease across the Acute/ Community Continuum, and within the primary care sector.
David’s major achievements or contributions are significant contributions to the consolidation of Community & Primary Health Services, Metro South HSD.

Gaylene is currently CEO of SouthEast Primary Health Care Network, and has held this position since July 2008.
Gaylene has worked in various executive, business development and educational roles within the non government primary care sector for the last 19 years. As a Registered Nurse she specialized in Palliative Care for several years.
She has led major organizational mergers and business transformation strategies, as well as developing new business initiatives and revenue sources.
Gaylene is an Honorary Visiting Fellow at QUT and was instrumental in the development of Brisbane Leg Clubs based on the Lindsay UK Model, which is part of an international research project through QUT. Gaylene was curriculum co-author of the QUT Community Practice post graduate course and presented educational consultancies for the Hong Kong Nursing Authority on nursing leadership, domiciliary nursing and palliative care.
Gaylene has been involved in the National Aged Care Alliance (NACA), ACQ Community Care Committee, UQ and QUT Post Grad Community Nursing and Aged Care Advisory Committees. Board positions she has held include the Australian Council of Community Nursing Services [ACCNS] Management Committee, Council Member of Aged Care Queensland and Board Member of Anglicare Australia. Gaylene is currently on the RCNA National Advisory Council and Community & Primary Health Care Faculty Advisory Committee.

Debbie Cowan is currently the Manager of the Logan-Beaudesert Health Coalition. Debbie holds a position as Adjunct Research Fellow in the School of Human Services, Griffith University, and is pursuing a PhD in the area of ‘leadership in a changing health system’. Ms Cowans undergraduate qualifications include a Bachelor of Health Science Nutrition and Dietetics (Hons) and studies in Health Management.
Prior to this, Ms Cowan’s role within Queensland Health, encompassed a variety of management, project management, health planning, clinical and population health roles which have provided her the opportunity to address local and strategic population health issues, and provide leadership to a range of stakeholders from government and non-government organisations.
Before joining Queensland Health, Ms Cowan, worked as a Spatial Data Specialist for a private firm, and was involved in the project management of a number of national and international contracts to ensure they were delivered on time and to quality standards.
Director Allied Health Services, Mater Health Service
Anne Maree commenced work as an occupational therapist in 1984 and worked in a number of hospitals and community agencies in Melbourne in Mental Health and vocational rehabilitation.
In 1992 Anne Maree relocated to Brisbane, and commenced work as the Director of Occupational Therapy at Princess Alexandra Hospital. During the next fifteen years, Anne Maree undertook several executive roles within the Princess Alexandra Hospital including, Director of Allied Health, Director Clinical support Services, Director Corporate Services and Director of Master and Service Planning. In 2006, Anne Maree took up her current position as Director, Allied health Services at Mater Health Services.
Director Health Promotion, Brisbane Southside Population Health Unit
Aleesa has over 10 years experience in local, regional and statewide population health and health promotion across a range of health issues. She has worked in a variety of areas, including alcohol, tobacco and other drug prevention, health promotion in schools, skin cancer prevention and chronic disease primary prevention. Aleesa has expertise in the areas of project management, program delivery, planning and evaluation, and manages a team of health promotion professionals working across various health issues and settings in the southside of Brisbane.
Aleesa is passionate about reducing health inequalities and promoting health through action which targets broader social factors such as education, the environment, income and community belonging.

Community Care Manager, Blue Care, Brisbane
Cathy has a nursing background and specialised in both oncology and cardiology before commencing work with Blue Care in 1991. Her work within Blue Care has spanned across both clinical and management roles as Learning and Development Manager, Director of Nursing for a 185 bed aged care facility and more recently as Community Care Manager for the Brisbane area which she took up in 2006.
With a focus on service integration through contemporary models of care, Cathy is passionate about the work Blue Care does across the continuum of care, including the promotion of self management models and chronic disease management.
