The Lifescripts initiative provides General Practice with evidence-based tools and skills to help patients address the main lifestyle risk factors for chronic disease: smoking; poor nutrition; alcohol misuse; physical inactivity; and unhealthy weight. The initiative assists with the provision of tailored advice to patients on modifying their lifestyle.
Prevention of Type 2 Diabetes & Lifestyle Modification Programs
This is the final year of the Lifestyle Modification Program (LMP) in its current form.

In order to continue to provide patients with options to improving their health through lifestyle modification, the following is an alternative program in your area:
Type 2 diabetes is a largely preventable chronic disease that is becoming increasingly common in Australia. If undetected or poorly controlled, type 2 diabetes can result in acute and long term complications.
Clinical trials have provided strong evidence that progression to type 2 diabetes can be prevented or delayed with lifestyle modification. Fact Sheet: Evidence to support lifestyle modification programs.
A key component of the Prevention of Type 2 Diabetes Program is the ability for a GP to refer eligible patients to accredited, subsidised Lifestyle Modification Programs (LMPs). To be eligible for referral, patients must:
What is involved in an LMP?
LMPs are group-based behaviour modification interventions that support patients at high risk of diabetes to undertake sustained lifestyle and behaviour change to prevent or delay the onset of developing type 2 diabetes.
LMPs involve a series of group education and motivation sessions supporting lifestyle change. Nationally accredited LMPs are required to meet the standards for subsidised Lifestyle Modification Programs for people at risk of type 2 diabetes.

Determining Risk
The completion of the AUSDRISK and a high score, 12 or more, are mandatory for the patients to be referred to a subsidised LMP. It may be completed by either the patient or with the assistance of a health professional.

Which MBS items can be used for assessment and referral to an LMP?
The time based health assessment items can be used for conducting a Diabetes Risk Evaluation for 40-49 year olds or a 45-49 Year Health Assessment:

Referral forms and completed AUSDRISK forms are to be faxed to SEA-GP (Brisbane) 07 3823 1408. We will then contact the patients and allocate them into appropriate LMPs according to venue, day and time.
New Lease of Life Campaign
The national marketing campaign has been developed to compliment the work of Divisions of General Practice in the implementation of the Prevention of Type 2 Diabetes Program with the aim of increasing the uptake of LMPs.
The campaign’s target audience is people aged 40-49 who are at risk of developing type 2 diabetes within the next five years, making them suitable for an LMP.
The campaign includes TV advertisements, radio community service announcements, print media (posters, brochures, wallet cards) and a microsite hosted on the AGPN website.
The key messages of the campaign are that LMPs can greatly help people who are at risk of developing type 2 diabetes turn their situation around and take control of their own health. The first step is seeing their GP.

NEW Online Active Learning Module (ALM)
Prevention of Type 2 Diabetes: Identification and Management of High Risk Patients
The Australian General Practice Network (AGPN) has released this six hour ALM consisting of three modules, where participants are guided through an interactive learning environment that features video, audio and a variety of interactive components including pre/post knowledge and skills tests, self assessment, evaluations and various interactive activities.
