The aim of the IM/IT Program is to:

  • To improve quality of care in the SEA-GP (Brisbane) region through the appropriate use of information management and information technology
  • To optimise the use of IM/IT which enhances information and services to GPs, other health care providers and the community
  • To use IM/IT to measure primary health care and the Alliance outcomes
  • To provide and maintain an efficient IM/IT infrastructure at the Alliance

The following services and resources are available to SEA-GP (Brisbane) members:

  • Data Quality - Providing guidance and assistance for practices to manage and utilise quality data to inform clinical and business decisions
    - Recalls and reminders
    - Data quality review for process improvement and business decisions
    - Utilising templates to deliver quality information for continuity of care management
    - Utilising Electronic Decision Support (eDS) resources

  • IM/IT Systems - Providing guidance for information management and technology requirements including:
    - Options for available technical service providers for systems purchases, upgrades and/or management
    - Options for training service providers
    - Information systems security and disaster recovery management
    - Secure electronic communications (PKI registration)

  • Health Informatics Standards – Providing guidance on developments in the health informatics standards arena

Referrals and Templates

SEA-GP (Brisbane) provides a range of resources and links to external websites for community services and hospitals and private practitioners and allied health with the aim of assisting general practice to enable continuity of care for patients.

GPAC Continuity of Care Planning Framework
The Continuity of Care Planning Framework was developed by GPAC to guide and outline best practice continuity of care planning processes. A copy of this framework and an accompanying manual can be found at the GPAC website.

An updated version of the Generic Referral Template  for Medical Director is available here (download information), if a more specific template is not available below.

Note: Referrals can now be sent electronically and securely, directly from within clinical software packages, using Medical Objects or similar methods, and signed using an individual PKI dongle. This minimises the extra handling normally associated with sending referrals, and results in a saving of time and resources previously needed in printing/faxing etc.
The iHealth Care directory is also available, providing information on contact details for GPs, Specialists, Allied Health, etc.

Community Services and Hospitals

A list of resources and templates are provided for your reference and usage which relate to primary health care services for local hospital or community service oganisations.

Where available templates are provided for download to incorporate into a range of clinical software packages.  Please refer to the relevant installation instructions for individual templates or the generic Instructions for Installing and Using MD Templates [PDF format; 169Kb].  Should you be using a clinical software package that a template is not available for please contact the IM/IT Program Manager to see if alternative options are available.  If you require assistance installing any of the templates listed below please contact the IM/IT Program Manager at the Alliance.

Mater

Referring to Queensland Health's Metro South public hospitals

Queensland Health has set up a dedicated website for GPs referring to Logan Hospital, PA Hospital, QEII Hospital and Redland Hospital. The site is now available at: www.health.qld.gov.au/metrosouth. It contains key telephone and fax numbers, referral requirements and additional information to ensure your referral is quick and simple.

NOTE: Waiting times can be extensive for some specialist areas. Provision of specific clinical information assists with triaging and assessment/management at the initial OPD consultation, plus less need for follow up visits, which means more capacity to see new patients and reduced waiting times.

NOTE: Information on average waiting times for Medical Speciality and Allied Health OPD Clinics at Redland Hospital is available here (password required).

Specific referral templates for sending by fax have been developed for the following OPD Clinics to ensure appropriate triaging of patients and follow up assessment and management

Private Practitioners and Allied Health

The Clozapine GP Shared Care Review Form template for MD3 and MD2 are available here.

This is a trial version only and will be refined as we receive feedback. 

Please note this is only applicable for GPs currently caring for consumers taking clozapine within a shared care arrangement with the PA Hospital Mental Health Service.

SEA-GP (Brisbane) is not able to provide direct IM/IT technical support. There are a range of companies in the Brisbane metropolitan area are available to provide IM/IT technical support services on a commercial basis, a number of which have a range of experience with working in the primary care setting.

SEA-GP (Brisbane) recommends that you seek guidance in respect to the quality and customer focused services of IM/IT technical support vendors from:

  • Consulting your peer practices; and/or
  • Consulting your software vendor.
